Scalpel (aka The Doctor) was a Decepticon Surgeon[2]. Of course, being a Decepticon, the term Doctor could easily double as a Madman, Murderer, and a master in all forms of torture. Scalpel is all of this, despite being tiny even by human standards. He, for some reason, enjoys taking rides to patients in the mouth of Decepticon Ravage.

Transformers: Revenge of the Fallen []
After Ravage defeated the NEST Soldiers and retrieved the shard, as the Microcons go back inside Ravage, Scalpel ejects himself, grabs the shard, and goes back inside Ravage with the shard to their next destination, although it was off-screen.
When the Constructicons and Ravage arrived at the Mariana Trench, they descended to find the body of Megatron. In hopes of him being able to save their leader, Ravage deployed Scalpel. Then he examined the damage and informed the Constructicons that he needed parts, and to "Kill Ze little One". Then Mixmaster and Long Haul then ripped the poor Scrapmetal apart. Scalpel then used the AllSpark Shard to revive Megatron, who burst back out the sea.
Following the capture of Sam Witwicky, the Decepticons hoped to get the meanings of the symbols he was seeing out of him. Scalpel transformed and arrived to work on Witwicky, sending his assistant inside the human.
As soon as Optimus and Bumblebee arrived to rescue Sam, Scalpel immediately hides and was able to escape during the shootout.

Scalpel linked up with Bludgeon and hid himself inside the larger Decepticon: once Bludgeon was captured by NEST, Scalpel emerged to free him and all the other Decepticons in human captivity.

- He resembles a Praying Mantis.
- His assistants are: Insecticons and Lensmeter Decepticons.
- Scalpel is great at fixing Decepticons as when he fixed Megatron and the other Decepticons that died in Mission City by using the AllSpark Shard with it's energy, it sends out energon waves making Megatron, Bonecrusher, Brawl and Blackout come alive.
